This document names someone to oversee your medical decisions in the event that you become incapacitated and are unable to make decisions on your own. He or she will act on your behalf when it comes to your medical care.
Should you become incapacitated, you will want your financial affairs handled as well. In this case, you would name someone as your durable power of attorney. This document is also included in an estate plan. It allows the person you name to make decisions about your finances, including real estate and other business matters, until you are able to do so again. Remember, if you have an estate plan in place, your wishes will be carried out according to it. If you do not, then the courts are left to decide who gets what.
